Insurance & Billing

You will be asked to present your insurance card with each visit. Please advise the receptionist of any changes in address, phone number, insurance coverage, marital status etc.

Financial Policy

There is a fee for our services. We participate and file claims with the following companies:

• Medicare
• Wellmark BCBS
• Iowa Medicaid/Medipass
• UnitedHealthcare of the River Valley
• Mercy Priority Health Network
• Mercy PHO Contracts
• Midlands Choice
• Iowa Health Solutions
• UnitedHealthcare – effective date 7/15/2008

Note: We will send in an insurance form for your insurance company even if we do not participate, but the patient will be required to make payment at the time of service. Co-pays are due at the time of service.

Your health insurance is a contract between you and your insurance company. It is your responsibility to pursue unpaid claims promptly.

For your convenience we accept Visa, MasterCard, personal checks and cash as a means of payment. 1.5% per month interest is charged on balances over 60 days. Monthly payment plans are available and can be arranged by contacting our billing department. Our staff will be happy to answer your questions regarding insurance, account balances and payment plans.

Cosmetic Procedures

Payment is to be paid in full 10 days prior to the procedure.
